Health & Wellness

The Cameron County School District recognizes that student wellness and proper nutrition positively impact students’ well-being, growth, development, and readiness to learn. A school environment that promotes student and faculty wellness, healthy habits, proper nutrition, nutrition education and regular physical activity is an essential element of the total learning experience. In order to create this environment, the school nurse’s provide health screenings and assessments as mandated by the State of Pennsylvania, refer students to health care providers for treatment and follow-up, provide care to students who are ill or injured, and educate the students about wellness and caring for their health.

We also work closely with the athletic director, athletic trainer, and the various sports coaches to maintain compliance with Pennsylvania Interscholastic Athletic Association (PIAA) health policies.

Cameron County School District Health Forms

Student Health History – This form is used upon initial admission to the Cameron County School District. 

Annual Student Health Update – Every year this form should be updated and returned to the school nurse to assure we have current information on your child’s health status as well as the best way to reach you in the event of an illness or injury.

Private Physical Exam Form - As part of the health services mandated by the Pennsylvania School Health Code, each student upon first entry, or in Grades K, 6, and 11 are required to have a physical examination. This exam may be done by the student’s family physician or by the school physician. Physicals completed up to 4 months before the start of the school year will count toward meeting this requirement.

Private Dental Exam Form - As part of the health services mandated by the Pennsylvania School Health Code, each student upon first entry, or in Grades K, 3, and 7 are required to have a dental examination. This exam may be done by the student’s family dentist, mobile dentist, or by the school dentist. Dental exams completed up to 4 months before the start of the school year will count toward meeting this requirement.

Medication Administration Consent Form In accordance with school policy, medication(s) should be given at home before and/or after school.  However, when this is not possible, prior to receiving the medication at school, each student must provide the school nurse with a Medication Administration Consent form signed by the student’s parent/guardian and a Medication Order from a licensed prescriber.  All medications must be in an original prescription bottle/container from a pharmacy. 

Diabetes Action Plan Individual Healthcare Plan to be completed by physician and signed by parent.

Allergy Action Plan Individual Healthcare Plan to be completed by physician and signed by parent.

Asthma Action Plan Individual Healthcare Plan to be completed by physician and signed by parent.

Medical Plan of Care for School Food Service  Individual Healthcare Plan to be completed by physician and signed by parent of student whose disability requires modification in provided school meals .

Seizure Action Plan Individual Healthcare Plan to be completed by physician and signed by parent.

PIAA (Pennsylvania Interscholastic Athletic Association) CIPPE (Comprehensive Initial Pre-Participation Physical Evaluation) All students planning to participate in athletics or cheerleading and a parent/guardian must complete sections 1 - 6 of this form annually prior to their sports physical. Section 8 must be completed by the student and parent/guardian prior to competing in additional sports during the school year.
